1. As a professional body it is important that we keep up to date information on our members so that we are able to provide relevant services.

2. All information is completely confidential and is captured and stored in a manner that ensures, to the best of our ability, the security and privacy of your personal data.

3. There are certain categories of information that we are required to collect from members in support of our application for professional body recognition under the auspices of the South African Qualifications Authority (SAQA). This includes information on home language, disability and socio-economic status.

4. What makes this application complete:
• Complete all sections
• The form must be signed
• Email certified copies of your Identity Document, Matric Certificate (where required), qualifications, professional
       memberships, and employment contract (a letter of confirmation of employment may substitute your employment contract)
       to info@epa-africa.com.
• Certificates must indicate qualifications and subjects passed
• Email a comprehensive CV to info@epa-africa.com
• Completed employer’s / referee’s authentication
• Completed undertaking by the applicant

5. Section 4 of the application is only required to be completed by those individuals wishing to apply for the (EP)SA designation.

6. A Consent and Indemnity form may be issued for completion post the application review.

7. Policies and codes of conduct developed and passed by the EPA may be issued at a later stage for attestation by members.

8. We appreciate that the completion of this form and provision of supporting documentation may take time to complete but do reassure you that once this information is captured, we will only need to manage changes to details if and when these occur.

9. As the Association develops and finalises supporting membership policy and conduct codes, these will be communicated separately and may require attestation by members.
