To be considered for this program, please fill out the following application and email a PDF version of your resume to PlanSacValley@gmail.com by Friday, October 22, 2021 at 5pm. The program cost is $80.

This application will determine your acceptance into the program and will help us find the best possible mentor match for you. Please note, this is a highly competitive program, so please be as detailed as possible and provide us with any information that may be helpful as we conduct the selection and matching process. This program strives to find individuals that will be dedicated to the program and create the best match possible for each mentor/mentee pair. PLAN Sac Valley is committed to creating a cohort that reflects and represents the diversity of our region.
