This form is for open age (18 and over) players to indicate their availability to play in the 2020 Hockey Queensland Championship.

You should only complete this form if:

You have a clearance from your registered Association/s; OR
The Association/s you are registered with is/are not entering a team in this year's event.

Players under the age of 18 may submit their availability, however, these players will only be made available to Associations at the discretion of HQ and when the list of 18 and over players has been exhausted.
