There is a room block established at the ROSE Hotel for Friday and Saturday at a cost of $181.00 + taxes/fees (Total cost per night = $206.34). 50% of the hotel bill will be covered by ASCLS-IL. Do you plan to stay at the hotel for those days? If so, please indicate your preference for room type. If you plan to stay outside the room block (Thursday or Sunday, kindly choose other and indicate your time frame below it)

Do you plan to attend the social event on Saturday evening from 5pm - 7pm at Axeclusive? If so, there is complementary transportation to and from the hotel - Do you plan to use the shuttle or drive to the social event venue? All attendees are required to sign a waiver to participate in the event activities.

Everyone attending and/or speaking at the fall meeting is invited to attend the ASCLS-IL board of directors meeting on Friday afternoon beginning at 12pm at the ROSE Hotel.  A plated lunch will be served.  In order to get an accurate count of the number of lunches, please indicate if you plan to attend the Board of Directors meeting on Friday October 13th at 12pm.  (Those indicating Yes from the survey will be sent a follow up email for their menu choice)
