On-Campus Unmanned Aircraft System Flight Request
The use of Unmanned Aircraft Systems (UAS) on campus is permitted on a case-by-case basis. University police or other campus personnel may request to see an approved copy of this form to ensure that you have received the necessary permission prior to operating a UAS, so please carry an approved copy of this form with you during your UAS operations.  

By completing and signing this form, you are requesting permission from the University of Utah to pilot or commission a pilot to operate an unmanned aircraft system (UAS/drone) on campus. You must submit this form at least 48 hours before your proposed usage, unless the Vice President for Administrative Services (or designee) provides a waiver. Decisions about UAS flights depend on the accuracy of information provided in your application.  If anything about your planned flight changes, you must submit a new request that reflects those changes and re-approval will be needed.

Submission of this form does not release you from any liability that may come from operating a Small Unmanned Aircraft on campus. You are liable for all compliance regulations related to drone operation, pursuant to FAA Small Unmanned Aircraft regulation (part 107). Submitting this form confirms your acceptance of liability for any infringements, damage, bodily injury, or other harm caused by operating a UAS outside of FAA and University regulations. Further, submission of this form is a request for approval for a flight and does not give you permission to fly at the time and place you have indicated. You will be notified if your request is approved or denied with-in two business days of your request.
