Food Provider/Vendor Survey (businesses, farms, restaurants etc)

Cape Cod Cooperative Extension, in an effort to assess the local food system, invites you to complete this brief five-minute survey.

This work is part of a greater food equity plan with a shared goal to streamline communications and relationships among both food retail and food donation resources.  Your insight and feedback will assist in identifying gaps in the flow of food in our local food system.
