TRIAL REGISTRATION FORM.

A RESPONSIBLE ADULT - PARENT OR GURDIAN - IS TO FILL IN THIS FORM.

THIS FORM IS FOR INFORMATION PURPOSES ONLY AND IS NOT A COMMITTMENT FROM OUR CLUB TO SIGN ANY PLAYERS NOR IS IT A REGISTRATION FORM FOR OUR CLUB.

To save time on the trial days, this form can to be completed and submitted in advance of the trials, but players will still be required to book in on both days.

Trial dates are Sunday's 8th & 15th June 2025 -

Under 7 / 8 / 9 / 10 / 11 / 12      9.15 AM BOOK IN   10.00 am start      60 - 80 minutes

Under 13 / 15 / 16 / 18    11.15 AM BOOK IN    12.00 pm start      80 - 90 minutes

SORRY BUT WE WILL NOT HAVE AN UNDER 14 or UNDER 17 TEAM.

Players Are To Be Ready To Play 15 Minutes Before Session Starts.

##  ALL THE ABOVE TEAMS WILL BE PLAYING IN THE SELKENT LEAGUE  ##

ALL PLAYERS ARE TO HAVE APPROPRIATE CLOTHING, FOOTBALL KIT, SHIN PADS, SUITABLE BOOTS AND / OR TRAINERS FOR BOTH GRASS AND ALL WEATHER PITCHES IF THE WEATHER IS TOO WET TO USE THE GRASS.

NO SHIN PADS = NO PLAY = NO EXCEPTIONS

Players to bring personal water bottles & PARENTS should have any required medication available.

ALL PLAYERS IN UNDER 16 AND BELOW AGE GROUPS MUST HAVE AN APPROPRIATE ADULT WITH THEM WHILST AT THE TRIALS.

PLAYERS ARE EXPECTED TO TURN UP FOR BOTH DAYS.

Toilets will be available.

Players must be under the age of the group they wish to play in on the 31st August 2025.
Players at the under 7 age group must be 6 years old on 31st August 2025 to play.
