The community feedback form can be used to:
-provide feedback on anything included in our code of conduct and safer spaces policy
-make a report or get support after the festival or any DCMF event

 You have the option to remain anonymous or not, but please note that we may need to be able to contact you for certain types of complaints should you wish us to follow up for detailed incident reporting or take action.

Your form response will be sent to conduct@dcmf.com, which DCMF's Executive Director monitors. Your identity will not be shared with any other staff or Board Members without your express permission.

If you wish to discuss a Code of Conduct Issue with someone other than the Executive Director, you may contact the Board of Directors at board@dcmf.com. Please note that every Board member has access to this address. If there is a particular board member you wish to speak with privately, please email this address and request their contact information.

We recognize that creating safer cultural spaces is an ongoing process of work, learning, listening, evaluation and action. As such, our Code of Conduct and policy is a living document that changes and grows as we do.
