This application is to be completed by any CGESD family requesting financial assistance to help pay for technology costs associated with obtaining household internet service for CGESD Distance Learning. The CGESD Internet Connectivity program is designed to provide financial support to families that do not have the financial means to secure internet for CGESD Distance Learning. This $100.00 stipend can be used for any of the following qualified technology expenses
(modem, router, monthly internet service costs, and other internet connection related expense(s). Complete only one form for each household; if you have more than one student attending our district, please complete the form using the child in the highest enrolled grade level in CGESD. outreach@cgesd.org or drop it off at our district office located at 220 W. Kortsen Rd
- You must complete the IRS W-9 form found after the application on the previous page

Details:
Limit one $100 stipend per household family. One time only stipend for SY 2021-22. Money is to be used for qualified technology expense(s). Funding is provided by the City of Casa Grande COVID-19 relief funds and will be paid from CGESD to applicant. By accepting these funds you certify that they will be used for qualified technology expense(s) outlined in this application.
