Gifted Academy Application

Please fill out the following form to apply for the O'Connor Gifted Cohort. Spaces are limited and will be considered in the order they are received. Applicants must have a gifted identification score at the 90th percentile or higher in any of the three areas tested on the CogAT: Verbal, Quantitative, Non Verbal.

If you are applying from a non-DVUSD school, please email your gifted testing results documentation to sara.stollaryates@dvusd.org or jennifer.swaine@dvusd.org.

If your student is currently in a DVUSD gifted program, you DO NOT need to enter gifted scores. If your student attends a school that is not part of DVUSD, you will need to enter scores.

Are you applying for a full application for gifted (90th+ percentile for Verbal) *

B - my child has scores of 80-89 in Verbal and we are requested a provisional placement (based on space in the program and student performance)
