This form is to be used to request REVISED transportation for the 2020-2021 School Year for a Westerville City School (WCS) student who will be attending DACC while WCS is holding in-person learning according to our cohort A/B system (Scheduled to begin Monday September 21, 2020).

Before proceeding further please make sure that you have identified the following:
- The student's WCS Student ID number (starts with a 3).
- The student's WCS cohort identifier (A or B).
- The student's DACC cohort group identifier (Blue, Green, or 5 Day Program)
- The student's lab level at DACC (level 1 or level 2)
