COVID Reporting
To help us track COVID cases and exposure events at St. Mary's School, please complete this form if your child has experienced any of the following:

Tests positive for COVID
Exposure to a positive COVID case (close contact within 6 feet for at least 15 minutes)
Develops COVID like symptoms
Exposure to someone with COVID like symptoms (close contact within 6 feet for at least 15 minutes)

Please complete a survey for each student that has experienced any of the above events.
