Athletic Consent Form (Annual)
This form governs all clubs, activities, events, intramural offerings, and interscholastic teams at the William S. Cohen School and the James F. Doughty School. This form is required annually.

Parent/Guardian: please review with your student and fill out completely. A Parent/Guardian and the Student will each sign electronically at the bottom of the form. 

Those wishing to tryout for / participate in an interscholastic sports team should begin by reviewing the Bangor Middle School Athletics Interscholastic Sports Guidelines & Agreement Criteria.

It is the responsibility of the parent/guardian, the student, and the coach or advisor to understand the behavioral, expectations and the supports and protections related to participation established in the following Bangor School Department policies: JFC- Code of Conduct; JFCI- Chemical Health; AC- Non-Discrimination & Affirmative Action; ACAD- Injurious Hazing; JICK- Bullying in Schools; as well as athletic and extra-curricular eligibility requirements, protocols, selection criteria, and the team-based expectations as communicated by the coach or advisor.
