Jazz & Beyond 2023 Performer Registration
Performers in Jazz & Beyond 2023 (August 11-21), please fill out this online form.  We may use the information from this form, plus images and videos, for publicity, festival program, sponsors, and grant applications.

1.  Name of ensemble or soloist (the way you want it to appear in the program) *
2.  Performance date, time, and location  (for example., "Mon., Aug. 14 - 2:00-4:00, Joe's Bar") *
(J&B staff will provide you details on parking, loading, and set-up time.)
3a.  Leader or contact person's name *
3b.  Leader or contact person's email address *
3c.  Leader or contact person's phone number(s) *
4.  Indicate number of performers, then list performers and their instrument(s) and/or voice (e.g., Jane Doe - guitar and vocal) *
5.  Performer or group's website, Facebook page, etc.  (enter "none" if there is none). *
You may enter more than one website, but separate names with semicolon.
6.  Provide a description of your ensemble or the performer, not to exceed 200 words, for J&B staff to use in publicity, website, festival program, etc. *
7.  Payment: name to appear on check and mailing address *
Payment will be on day of performance.

Please supply your own money box, starter funds, and inventory sheets.  You keep all proceeds.
9.  Do you need a W9 form to fill out?
If you need a blank W9 form, go to http://jazzcarsoncity.com/blankw9.pdf to download it.  Fill out the form and, for your security, mail it to Mile High Jazz Band, 191 Heidi Circle, Carson City, NV 89701.  We only need the first page.

10.  If the festival is providing audio/technical support, please provide a stage plot indicating location of performers, DI's, microphones, amps, etc.  Email to info@jazzcarsoncity.com (PDF or jpeg) or mail to Mile High Jazz Band, 191 Heidi Circle, Carson City, NV 89701.  Have you sent a stage plot?
