Purposeful
Movement is designed to improve physical fitness through dance exercises which
concentrate on the cardiovascular system, balance, coordination, muscle
conditioning and endurance. The routines include a required warm up period and
during the routines, participants are advised to pace themselves.
I
understand that this exercise program is strenuous exercise that may cause
physical injury or illness. I further understand that no representative of the
program is qualified to render any medical consultations and that participants
are to consult with a licensed physician concerning advisability of
participation in the exercise program. I acknowledge that information
concerning the program has been provided and that all my questions have been
answered in a satisfactory manner.
Accordingly, I hereby voluntarily assume all risks of
physical injury and/or illness known and unknown that may result from my
participation in fitness. I will attempt to follow instructions given but not
attempt any movement that causes pain or poses risks of injury to my body. With
this being stated, I warrant that I am in good physical condition and have no
disability, impairment or ailment which would be adversely affected by my
participation in this exercise program.
Further, I hereby agree to forgo, release, and forever discharge any
legal claim, action or proceedings against Purposeful Movement.
