This is only for students for which NO digital annual student forms have been updated. If you have questions about whether this has been completed for your student, please email us at publicrelations@myhhcs.org or text the student(s) name(s) to 937-463-3937.

Forms should be completed ASAP! It is critical that this form is filled out for EACH student in the district. Failure to complete the form in a timely manner could result in students being locked out of district accounts and/or district-owned technology.

PLEASE NOTE: This is for the online Annual Student Update form, NOT paper hardcopies turned in at the beginning of the school year. Even if paper hardcopies were turned in, we still need this form completed. Also, we did have a small subset of data that had errors in it upon initial submission. We apologize to those having to make a second submission.
