CLI Family/Provider/SSA Survey - 2020
CLI wants your comments all year, but we also send out at least 1 formal survey each year to collect ideas about what people like, dislike and/or wish for.

If you are helping a CLI client with their responses, please go to www.CLIsupports.com and click on "CLI client survey".  The questions are slightly different and we want to make sure that responses are tabulated in the correct demographic.

Be honest.  CLI gets better when we know what we should do more of-- AND what we should do less of-- AND what maybe we should immediately stop doing.

Every box on this survey will require an answer, but anything you type in will allow you to move on to the next question.  However, just remember we all learn better when someone takes the time to teach us.

Anyone who completes this survey by Tuesday, December 22nd 2020 (and includes their name & mailing address on the survey will get a free Ohio Lottery Ticket.  Who knows, this might really be your "hap-hap-happiest Christmas ever"!
