MAFA 2023 Instructor Travel Proposal Form
Please use this form to provide MAFA with your proposed travel to and from the MAFA 2023 Conference.
The Conference Advisory Committee will review your proposed travel costs and let you know if they are approved or if we have any questions. PLEASE DO NOT PURCHASE TICKETS UNTIL YOU RECEIVE APPROVAL. If you have any questions, please email Chris Field-Funk at education@mafafiber.org.

Per your contract:

MAFA will reimburse Instructor for documented travel expenses as follows:

1. If traveling by air: 
      1. Least expensive round-trip coach airfare from city of origin to Harrisburg International - HIA (preferred), Philadelphia International - PHL, or Baltimore-Washington International - BWI, Airport.

        1. Air travel is to be arranged by Instructor after receiving confirmation from the MAFA Education Chair that the workshop has sufficient enrollment to run.

        2. Instructor shall obtain Education Chair approval of the flight arrangements before purchasing tickets.

      2. Cost of up to two checked bags.

      3. Cost of ground transportation from the Instructor’s home to the airport. 

      4. Cost of reasonable ground transportation from Harrisburg, Philadelphia, or Baltimore airports to Millersville University.

        1. MAFA will organize shuttles to and from Harrisburg Airport. In this case, MAFA will pay for the shuttles.

        2. If transport is necessary to or from Philadelphia or BWI airports, instructors will be encouraged to share rental cars or shuttles with other instructors. In this case, MAFA will reimburse the instructor renting the car or paying for the shared shuttle.

      5. Cost of airport meals, if appropriate (receipts required).

      6. MAFA will not be responsible for additional charges incurred in rescheduling flights and ground transportation if a reserved flight is missed due to error or negligence.

2. If traveling by car:

    1. MAFA will reimburse driving expenses, as follows, not to exceed the cost of airfare as defined in section 7(a):

      1. Mileage at the current U.S. government mileage allowance as of May 31, 2023;

      2. One night of hotel stay in each direction, not to exceed $150 each, if distance traveled is over 400 miles each way (receipt required);

      3. Tolls;

      4. Meals while traveling, not to exceed $50 per day (receipt required);

      5. Free parking will be provided at the conference venue.

3. Shipping Allowance:

    1. With prior approval from the MAFA Education Chair, MAFA will reimburse for reasonable shipping charges for workshop teaching materials.

    2. MAFA will not reimburse for shipment of materials the Instructor plans to sell.
