Ascension Illinois accepts requests for community investments to charitable organizations that align with our mission, values, organizational priorities and prioritized community needs as part of our commitment to our communities.
Ascension Illinois will seek to award requests that support external community health and impact programs, events and other community-based activities that align with our mission, organizational strategy and goals. Requests should aim to improve access to health care services, advance medical or healthcare knowledge, or enhance the health and wellness of the communities we serve. Preference will be given to programs and activities that provide a safety net to groups that have been historically marginalized or made vulnerable and/or address the respective hospital’s prioritized community needs, improve health equity and/or a social determinant of health.
This request form is for organizational support and not intended for individuals.
Requests for events must be submitted 90 days in advance of the event date.
Requests for this fiscal year must be submitted no later than April 1, 2025. Requests received between April 2nd thru June 30th, will not be reviewed until our fiscal year 2026 beginning July 1, 2025.
Please note: hospitals will only consider requests that are located within their service area and local community.
