Special Olympics Wisconsin & Forward Madison Unified Soccer Team 2023 Tryout Form
The Unified Soccer Series is a partnership between Special Olympics Wisconsin, Forward Madison, Special Olympics Nebraska, and Union Omaha. The series will include home-and-away matches between unified soccer teams in partnership with both USL clubs.

Participants must be at least 14 years old to attend the tryout. All athletes and unified partners that register for the tryout must have the appropriate paperwork on file with the SOWI State Office by Friday, March 31. The registration will close on Friday, March 31 at noon.   

By completing the form below, I agree to attend tryouts and give my full potential. I understand that this is a tryout and I will be notified at a later date if I am selected for the team. 

Season Schedule: 
  • Sunday, April 2 from 1pm - 4pm - Team Tryout
  • Practice Schedule TBD
  • Thursday, May 18 from 4pm - 9pm - Team Signing Day (players will receive tickets to Forward Madison game and be featured during the professional match)
  • Friday, June 23 through Sunday, June 25 - Forward Madison Unified Team @ Union Omaha Unified Team (unified game played in conjunction with a Union Omaha professional match on Sat, June 24)
  • Saturday, July 29 from 4pm - 9pm - Chicago Fire Unified Team @ Forward Madison Unified Team hosted at Breese Stevens Field in Madison (unified match followed by the professional game)
  • Saturday, August 12 from 4pm - 9pm - Union Omaha Unified Team @ Forward Madison Unified Team hosted at Breese Stevens Field in Madison (unified match followed by the professional game)

If I am selected for the team, I can attend the Unified Soccer Series away game at Werner Park in Omaha, NE on Saturday, June 24. I understand that with travel days, I will be out of town from Friday, June 23 through Sunday, June 25. *

If I am selected for the team, I can attend the Unified Soccer Series home game at Breese Stevens Field in Madison, WI on Saturday, August 12, 2023.  *

If I am selected for the team, I can attend the Team Signing Day at Breese Stevens Field in Madison, WI on Thursday, May 18 from 4:00pm to 7:00pm (will have the opportunity to attend the professional match afterwards)

Practice location will be determined after the roster is finalized to try to find a location that accommodates most of the team and coaching staff.
