SPA ACCREDITED MEMBERSHIP FORM
Are you a Wellness Centre, Spa, Fitness Centre? Then this is the right solution for you. This membership.
• Eligible to apply for accreditation.
• Spa Membership. For a stand-alone company of any sort, including a spa with one branch.
• Please note this membership fee is excluding the fee of the accreditation administration, accessor travel, cost & lodge fees (contact us for more).
• Once you pay for your MEMBERSHIP, our team will contact you to set up a date and time of the accessor.
• We strongly advise facilities to be ready to achieve accreditation once you purchase this membership, in order to avoid higher costs. To prepare to contact your SWAA Chapter Country or admin@swaafrica.org 
• All memberships run from June 1st – June 30th the following year. Memberships cannot be carried over if you join later than June 30th.
• Are you a member of a national association affiliated to SWAA?  Please contact us for what benefits you get from SWAA.

SWAA Membership Entitlement and Fee Structure
 The membership year runs through July 1st – June 30th. All new memberships will be valid through June 30th of each year.

* Areas on this document must be filled out with the correct information before sending back to us. If the information given is not correct, your application will not be void and you will have to start the process again.
Once your application is confirmed, you will need to send a confirmation of payment to us. Once payment is confirmed you will be sent a welcome email and issued a member log in code.
