This is a request form to be filled out by L.P. Collins staff. This is the first step to propose the PTA to fund a school initiative for the betterment of the school. All required fields must be filled out. If further information is needed, the staff member may be asked to attend a PTA Board Meeting to provide additional supportive material for the request.

The completion of this form does not guarantee funding will be provided. Several factors will be considered including but not limited to funds availability, magnitude of impact and timing.
