What size booth will you need? *

All participants are requested to contribute an item to a large gift basket.  Entry forms will be given to all guests for entering a drawing for this gift basket.  For completed entry form to be valid, guest must have form validated at all booths.  What will you contribute to basket?  (Item may be dropped off in advance or brought to event before start time.) *
Will you offer any special activity or special products at your booth that would be newsworthy for event publicity?  If so, please describe.
I understand that I will need to pay a $10-20 co-sponsor fee for event expenses and that I should have my booth set up by 4:30pm on the day of event unless special arrangements are made in advance *
