1) This form must be completed and submitted online.
2) A hard copy of the notarized Emergency Medical Form must also be turned in.
    Only when both forms have been received, along with the required registration fees, will your singer's
    registration be complete for the 2023-2024 season.
3) All forms must be submitted no later than one week after your singer begins rehearsals.
    After this time, placement for your singer will be temporarily suspended until form submission is complete.
