2024-25 Post-Secondary Qualifier (Waterloo) Registration Form
This is the official registration form for the Waterloo site of the Consensus 2024-25 Post-Secondary Qualifier, held at the University of Waterloo on Saturday, January 19. Please fill this form out once per team.

What school do you attend? *
Please list the names of the players on your team. You can change the composition of your team prior to the tournament, but please inform us if you do so.

Note that no team at the Championship may include any player who played for a different team in the Qualifier. Players who did not play in the Qualifier will still be eligible.

Instead of using ranked designators like A, B, and C, we are asking teams to choose names for the Qualifier and Championship in a similar pattern to sports team names. We will prepend a school identifier to your team name (so a team from the University of Waterloo named the "Geese" would be known as the "Waterloo Geese"). Team names should generally be about 1-2 words in length and cannot contain loaded or offensive language. If your team qualifies for the Championship, the same team name will be used there.

Please read and acknowledge the following terms.

1. The base entry fee for this tournament is $120 per team (before applying discounts for buzzers or travel). Payment can be made via e-transfer (contact@consensustrivia.com), cash, or cheque (made out to Consensus Trivia), anytime on or before the tournament date.

2. This tournament has a preliminary field cap of 10 teams, with a maximum of 3 teams per school. This cap will be expanded once we can confirm rooms, staff, and even numbers of teams.

3. The Guaranteed Registration deadline for this tournament is January 5. Any teams that register on or before that date are guaranteed spots, subject to our field cap and provided we have enough staff. We may or may not be able to accommodate teams that register after that date.

4. The Late Drop deadline for this tournament is January 5. Any teams that drop out after this date will still have to pay 50% of their registration fee.
