Woodland Hills Transportation Form
To ensure compliance with State regulations governed by Act 372 of the PA School Code the Woodland Hills School District requests the parent/guardian for each student desiring daily transportation or periodic transportation to submit a request for service annually. This procedure is in addition to the student’s school sending enrollment notification forms and rosters verifying enrollment.
Please complete this transportation form for each student for the 2023-2024 school year. Allow 5 business days for requests to be processed. If your child does not ride for a week or more the stop could be eliminated and a new form would need to be submitted.
