Change of Distance Learning Plan Request
If you need to change the method of distance learning for your child, please complete and submit this form. A few details regarding changing formats and timelines for implementation of new format:
- Changes from offline to online may be implemented immediately.
- Changes from online to offline will have a slight turnaround time as we get a paper packet ready for your child to be sent via email or picked up in person depending on your selection below. An administrator will be in contact with you regarding this change and when the first packet will be ready.
