Bydd angen llenwi'r ffurflen yma mond unwaith ar gyfer y cyfres cyfan (gan gynnwys unrhyw rhagchwiliadau cyn-ras). Nid yw llenwi'r ffurflen hon yn eich ymrwymo i gymryd rhan yn unrhyw un o'r rasys a does dim angen talu ar hyn o bryd.

Consent and registration form for the WFRA South Wales Junior Series (including the Welsh Champs and pre-races recces). Details of the races here: https://sites.google.com/site/welshfra/ieuenctid-juniors/de-cymru-south-wales 

You only need to fill in this form once for the whole series (including any pre-race recces). Filling in this form does not commit you to entering any of the races, nor is there anything to pay now.

The races are free for WFRA junior members (£3 for non-members). You can join the WFRA here for £5:  https://www.fabian4.co.uk/default.aspx?EventID=3237 (open to all, in Wales or beyond).
