Urgent request: Please fill out applications in accordance with the requirements:

  • The application must be completed ONLY in ENGLISH using LATIN letters.
  • A NEW application must be created for EACH number.
  • The name of the participant must be written in the format "first name-last name" and in ALL CAPS: JASON STATHAM
  • The duration of the number must be indicated in the format: 03:10
  • The age must be indicated by a number: 7. If there are several participants, then: 7-15
  • City and Country must be indicated: Prague, Czech Republic

All subsequent documents (Diplomas, certificates and letters) of thanks are issued on the basis of the data that you specify in the application. IT IS IMPORTANT - if you entered your data incorrectly, and we were not informed about it in time, the rework of the diploma will cost 7 Euros. Submission of the application means acceptance of the Regulation.
