Self Evaluation: Accounts Receivable Specialist

This Google Form is to be submitted by the Accounts Receivable Specialist who is due for their Performance Review (you) prior to your In-Person Performance Review. The Regional Director of Operations will also complete an employee evaluation form. During the In-Person Performance Review, you and the Regional Director of Operations will discuss where your evaluations align and where adjustments or further development would be beneficial.

You will be evaluating yourself on your exhibition of Forest Family Dentistry's Core Values and your performance of the Accounts Receivable Specialist Primary Duties. Additionally, you will share the professional and personal goals you want to achieve over the next year. At the end of your In-Person Performance Review, you and the Regional Director of Operations will agree upon expectations for the following year to help you achieve your goals.
