Our school would appreciate your assistance in completing the following survey  as honestly as possible. We are gathering information through this survey about the kinds of programming and services that would be most beneficial for our students, their parents, and our community through the 21st Century Afterschool Grant.  All data collected will be compiled and used to make decisions about the types of services that the grant can offer so that our students can grow to become capable members of society.  All responses are completely anonymous.
