2023-2024 Attendance Appeal Form
This completed loss of credit appeal form must be submitted within 10 days of report cards posting to Genesis each quarter. Any supporting documentation that may excuse a student’s absence (medical, college visit, etc.) must be submitted to the MHS Office of School Counseling before the appeal is scheduled. You will be notified regarding the approval, conditional approval, or denial of your appeal via email from the Attendance Appeal Committee.
