This form is for use by everyone coming to Crossroads Presbyterian Church.  Please complete and submit prior to entering the building for work, meeting, study, or other activity. Completed forms will be held in a Crossroads secure file and safely destroyed after 30 days.
While Crossroads is committed to the health and safety of all those that enter its building, we are also aware that taking all of these precautions do not, in and of themselves, guarantee that COVID-19 will not enter our campus. By coming onto the campus, you are agreeing to comply with Crossroads' policies and procedures, and further understand and agree that Crossroads has taken all reasonable measures, to the best of its ability, to create a safe environment.
