FR Parent/Guardian ONLINE Learning Survey for the 2021-22 Academic School Year

For the 2021-2022 school year, Franklin Regional School District will offer two learning environments. The first will be fully in-person five days a week. Please note that we will be able to accommodate ALL K-12 students in person for next school year. The second option will be a fully online synchronous environment five days a week. There will be no hybrid option offered.

All students are assumed to be returning to school for in-person learning unless this form is completed .

Please complete this form if you would like your child to be enrolled in a fully online synchronous learning environment for the entire 2021-2022 school year. If you have more than one child, please complete the survey again for each additional child.

THIS SURVEY MUST BE COMPLETED BY FRIDAY, JUNE 11, 2021 TO ENROLL IN THE ONLINE LEARNING PROGRAM FOR NEXT SCHOOL YEAR.
