Caldwell Middle School Schedule Change Request Form
Schedule changes will be made the first two weeks of school.  Please read the following on how the process will work:  After 8/13/20, schedule changes will not go into effect until AFTER 8/19/20.  Once the schedule has been changed, the student will receive notification.  Schedules will be able to be changed through this form for the first two weeks of school.  Please refer to the list of courses on Mrs. Philley's Counselor Corner if needed!  There is not a guarantee that a change can be made. The last day to make changes for the 20-21 school year is SEPTEMBER 2nd.
