Health Equity Award Nomination Form
We are seeking individuals and organizations in Greater Worcester who are working toward the goal of health equity – ensuring that all members of our community should have the opportunity to make the choices that allow them to live a long, healthy life, regardless of their income, education, or ethnic background.

Following are criteria for consideration. The nominee must:


- Exemplify excellence in work toward the goal of health equity
- Integrate the three core principles in all efforts:
          1) Invest first in the community
          2) Empower, listen to, and respect community voice
          3) Eliminate gaps between services

The deadline is Febuary 21st 2025. Nominations will be considered and voted on by the Steering Committee of the Coalition for a Healthy Greater Worcester. Winners will be announced at the Coalition's Annual Meeting in March 27th 2025.
