Thank you for your interest in admitting your child /children to Al-Fath Madrasah classes at Al- Fath Trust community centre.
Please fill in this form in BLOCK CAPITAL LETTRES. Please ensure you understand the Rules and Admission Requirements before completing your application.

PLEASE FILL IN ONE APPLICATION FORM PER CHILD SEPARATELY 

All forms must be submitted with the relevant signature(s). Any forms without a signature(s) will not be processed.
We anticipate that there may be a significant demand for places and thus our policy may be to prioritise places to children who are currently not attending any Madrasah/classes elsewhere.
Please note completion of the application form does not at this stage guarantee a place due to limited numbers initially.
