Please evaluate honestly. DO NOT just give 10's if the student does not deserve that score. For each area, please score the student between 0 and 10.

PREPARATION: Did the student come prepared to minister to those around them? *
COOPERATION WITH PRACTICUM LEADER: Did the students show a willingness to work with you and learn about your ministry area? *
COOPERATION WITH OTHERS: While the student was involved with your ministry area, did the student have an attitude of cooperation and working together with others in your ministry area? *
DESIRE TO LEARN: Did the student show a willingness to jump in and learn even if this wasn’t an area they were personally passionate about? *
ACCEPTANCE OF CORRECTION: Was the student willing to be taught and accept correction? If correction was given, did the student respond with humility & grace? *
GOSPEL INTEGRATION: After working with you, is it apparent that the student grew in their knowledge and understanding of your ministry area, and how your ministry area is an effective way of reaching people with the gospel? *
LEADERSHIP SKILLS: Did the student show that they had the ability and passion to lead others, in both example and in word? *
