1. I authorize Orleans/Niagara BOCES and/or my child’s home school district to conduct collection and testing for COVID-19 through a nasal swab, ordered by an authorized medical provider or public health official, of my child. I understand that such testing may occur either at BOCES or at my child’s home school district. I understand that this testing is voluntary and that my child is not required, nor am I required consent for my child, to undergo such testing as a condition to attend school.

2. I give consent for my child's test results to be disclosed to Orleans/Niagara BOCES, my child’s home school district, and any county, state, or other governmental authority as required by law (including the local health department).

3. I acknowledge that a positive test result will require that my child be sent home from school and remain at home until my child meets the criteria to return to school according to the local health department.

4. I understand that signing this form and giving consent for my child to receive COVID-19 testing does not create a doctor/patient relationship. I understand that neither the BOCES nor the home school district is or will be acting as my child's medical provider. I understand that testing does not replace treatment by my child's medical provider. I accept complete and full responsibility to take appropriate action with regard to my child's test results. I agree that I will seek medical advice, care, and treatment from my child's medical provider if I have questions or concerns, or if my child becomes ill, or if my child's condition worsens.

5. I understand that, as with any medical test, there is the potential for false positive or false negative test results to occur.

6. I understand and agree that this consent form will be valid through June 30, 2021, unless I revoke this consent in writing. I understand that my child may be tested multiple times during the 2020-2021 school year.
