The Home-Based Care Paradigm: Perspectives and Priorities of Hospital and Health System Leaders
As a new way to enhance the experience for patients and providers, the paradigm is shifting care delivery from the hospital in to the home. With the accelerated pace of advancement of medical technologies and services that monitor, manage, and measure patients at home - what does this new model of care mean for health systems and clinicians? We want to hear from you and what matters the most.

In appreciation of completing the survey, you will receive a summary of the survey results. Additionally, all survey participants that are currently employed by a hospital or health system will receive a complimentary registration to Becker's 8th Annual Health IT Conference or Becker's Hospital Review 14th Annual Meeting. If you will not be attending, you are welcome to designate a hospital or health system employee to attend in your place.
