PARENT CONSENT TO IN-SCHOOL COVID TESTING
Please complete a separate form for each of your children.

Diagnostic testing for children who present with COVID symptoms after their arrival at school will be available through your School Nurse.  Students without significant illness will remain in school with a negative test result.

Also available will be testing for students who may be identified by the Ontario County Public Health for quarantine.  Students identified for quarantine whose parents consent to COVID testing are able to continue to attend school (fully masked) and not be quarantined if exposed to a positive case. Your child will be tested daily for several days in this situation.  This in-school testing option is available through a partnership with the Ontario County Public Health.

For your information, the testing will be done with a table-top PCR unit, using a nasal sample (a Q-tip inside the nose for a few seconds, each nostril) - not the unpleasant, long, thin swab/brush used earlier in the pandemic.

Positive COVID testing results will be shared by your School Nurse.
