Touring Artist Required Report of Scheduled Services
Instructions:
Your sponsorship in the Children's Creative Project's Arts Catalog and website requires you to report all performances and workshops scheduled in public and private schools in Santa Barbara and San Luis Obispo counties.
Please note:
Please fill out this report when you schedule the services and before the service dates.
Artists must bill CCP for payment. CCP will bill schools and libraries and pay artists directly.
After providing the services, artists must send a SEPARATE BILL to the CCP for prompt payment. Schools must NOT pay artists directly.
Santa Barbara County public schools receive
one $600 Arts Credit per year
automatically applied to their total bill upon receipt of reports.
New services as scheduled may be reported throughout the year using this form.
