These forms must be completed at the start of each school year FOR EACH STUDENT. This is in lieu of the paper forms sent home to students in backpacks on the first day of school.  Thank you for your cooperation!

HANDBOOK AGREEMENT
The school handbook is available on our website (https://carver.org/carver-elementary-school-student-handbook-2020-2021/) so that you and your child can read and discuss it together. As an indication that you both have read this handbook, please complete the section below.
I, the parent/guardian, have reviewed the 2020-2021 Elementary Handbook and reviewed it with my student. *
USER AGREEMENT FOR PARTICIPATION IN AN ELECTRONIC COMMUNICATIONS SYSTEM

I, the student, have read Carver Public Schools' Acceptable Use Policy and Administrative Procedures and agree to abide by their provisions. I understand that violation of these provisions may result in disciplinary action including but not limited to suspension or revocation of privileges, suspension or expulsion from school, termination of employment, and criminal prosecution. *
I, the parent/guardian sponsor, have read Carver Public Schools' Acceptable Use Policy and Administrative Procedures and agree to abide by their provisions. I understand that violation of these provisions will be subject to appropriate disciplinary action and/or prosecution. *
MULTIMEDIA CONSENT FORM
I, the parent/guardian, authorize the Carver Elementary School to use and reproduce photographs and/or videotapes of my child and to circulate same for community awareness and education and publicity purposes. Such uses may include the following: school functions and school presentations, classroom displays, newspaper releases, cable television, class memory books, class photographs, and other similar school uses. *
I, the parent/guardian, authorize the Carver Elementary School to use and reproduce photographs and/or video of my child to be placed on the Carver Public Schools Website. Only group photographs, without student names, will be posted on the Carver Public Schools Website, except where additional parental permission is given on a case-by-case basis. *
