If you feel sick on the day of the event, or cannot respond truthfully to all questions below, please do NOT participate in the day's ride.

The State of Vermont currently requires that all event organizers collect and maintain participant information for each individual and each event. This information includes collecting data on potential exposure to Covid-19. Information must be held for 30 days after each event.

As per Vermont State, anyone returning to or traveling to Vermont must adhere to the mandatory travel guidelines and either complete a 14-day quarantine or a 7-day quarantine followed by a negative Covid-19 test before participating in any event. If you received your final COVID-19 vaccination more than 2 weeks ago, you are not required to quarantine. If you have not followed the above guidelines, please close the survey and do not attend GMBC events until you complete your quarantine or vaccination protocol.

Rides must adhere to an “arrive, play and leave” mentality. Groups may not gather before or after activities (no tailgating, etc.). Come to the ride properly equipped including pump (or CO2 cartridge), tire levers, spare inner tube, drinks and snacks.

To reduce close in-person contact, this form also requests emergency contact information that was previously collected using sign-up sheets at the ride start.
