Please use this form to book your JUNIOR SCHOOL childcare provision for both Before and After School Clubs for the period Monday 4th July to Thursday 21st July 2022 only.

PLEASE NOTE THE LAST DAY OF TERM IS THURSDAY 21st JULY AND SCHOOL IS CLOSED TO ALL CHILDREN ON FRIDAY 22nd JULY

Please note that on the last day of term, Thursday 21st July, there will be no After School Club provision. Breakfast Club will be running as normal

For After School Club, please ensure to indicate your intended collection time on each day your child will attend.

If you are unsure of your requirements for the whole period, please just complete the information that you currently require and then submit a further form at a later date. In this instance please make sure to complete section 9 of the form to advise if the information is in ADDITION to the information already provided, or an UPDATE.

If you require a late emergency 'on the day' booking, please do not complete this form. Please ring the School Office on 02380 261808 and, if calling before 3pm press 1 to leave a message, or if after 3pm hold to speak to a member of staff.
