If your child is sick, please have them check out by going to the nurse for evaluation.

This form is used for student drivers ONLY.

All early dismissal requests will be verified before a student is released. We will call the number listed in PowerSchool for said verification.

EARLY DISMISSAL FORMS MUST BE COMPLETED A MINIMUM OF ONE HOUR PRIOR TO STUDENT DISMISSAL.

You DO NOT need to complete this form if you, as a parent, are checking your child out. You will come into the school and be asked to provided identification before your child is released from school.

Please do not enter requests more than 3 days in advance.

As a rule, CCHS does not do checkouts after 1:45pm.

Late check-ins and early check-outs are recorded as tardies under the state Student Information System (SIS). Students who have excessive tardies (late check-ins or early check-outs) may be assigned ‘make-up time’ at the discretion of the principal. with consideration to lost educational time as described in section E, Excessive Absences and Tardies.
