Mobile Hotpspot/iPad/Equipment Loan Checkout Agreement

Important Information!

You must complete this Loan Agreement to check out an iPad, mobile hotspot, or related equipment from Nueces County Public Libraries. This agreement establishes your right to borrow the Equipment, your responsibilities for it, and the procedures that are followed in case of damage or loss.

The form is below, but do not skip following information. You will be held accountable to the terms listed below.

Loan Policy

*All patrons must agree to and sign a copy of this Agreement Form before checking out Mobile Hotspot/iPad/Equipment.
*The Nueces County Public Libraries reserves the right to refuse, deny or reject checkout privileges for any reason or for no reason at all.
*iPads and iPad accessories are available for checkout only to current Nueces County students (with student ID) or a parent or
guardian with a valid, fine-free library card that is at least 30 days old. The parent or guardian must have valid Nueces County
Driver’s License and Library Borrower Card.
*iPads are available for children 2 ages and up. iPads are loaded with apps for math, letters, games and more. The iPad may
not leave the building or the kids area.
*Checkout period: 2 hours in the library.
*Mobile hotspots and accessories are available for checkout only to current Nueces County residents with a valid Nueces
County Driver’s License and Library Borrower Card.
*iPads, mobile hotspots and Equipment are available on a first-come, first-served basis.
*Holds may not be placed on iPads or iPad accessories, and Equipment checkouts may not be renewed.
*No patron may check out more than one iPad at a time/per household.

Special Request

*iPads may be checked out to students in grades Kinder - 12th that are low-income. iPads are for school work during a pandemic and must be returned when the student returns to a school/educational setting or 6 weeks to be returned for a equipment checkup. (i.e. software update)

Return Policy *

I understand that the device(s) and equipment must be returned to the Circulation Desk at any NCPL location (or another designated location (curbside location); return iPads to check-out point), by the due date. Equipment, mobile hotspots, and iPads must not be returned in book drops. I understand the Library may use any appropriate means to collect the device(s) and amounts owed to the Library due to the failure to return it by the Due Date. I understand that the hotspot will be deactivated when it’s 1 day overdue.

Overdue Policy *

If the Mobile Hotspot is lost or not returned within 3 days after the due date, wireless service will be terminated and the Hotspot will become unusable. The due date is on the receipt give at the time of borrowing. I understand the Library may use any appropriate means to collect the Hotspot/device and amounts owed to the Library due to failure to return it by the Due Date.

Fees & Fines *

I understand that fines and fees will be charged if I return the items late or if they are lost or damaged and my library card privileges will be blocked until the Mobile Hotspots/device is returned in working order and outstanding fines are less than $10 on my account. Late fee for iPad is $25 a day (maximum fine $500) Lost or damaged iPad fees are $900 (iPad cost). Lost or damaged hotspot fee is $75 and $60 for hotspot cable/charger $15 case.

Internet Access *

I understand that when accessing the Internet through the Hotspot I am accessing the Internet through Teen Mobile & Sprint’s network and not the Library’s network. Use of the hotspot is subject to Teen Mobile & Sprint’s Acceptable Use Policy, Privacy Policy and Terms of Use, all of which are available on Sprint’s website (currently located at www.sprint.com/legal/agreement.html). Please read these documents before using the Hotspot. Internet access through the Hotspot is similar to Internet access at any NCPL location.

Data Responsibility *

I understand that the Library is not responsible for any files, data, or personal information accessed/transmitted using the Mobile Hotspot/device.

Borrower's Responsibility *

The Mobile/Hotspot device, iPad and equipment remains the Library’s property at all times. The Library may request that I return the Hotspot/device, iPad, and equipment at any time. I will never loan out the Hotspot/device, iPad, and equipment to other individuals, avoid damaging it, know where it is at all times and will not disassemble or attempt to repair it. If it is lost, stolen, or damaged, I agree to call or visit the Library to notify. I understand that the Library may choose not to replace or repair the Hotspot/device at its sole discretion. I agree to return the Hotspot/device (device, power cable, adapter and case) to the Library in the same good working condition as it was when checked out. I will only use the Library provided charger and that I am responsible for damage to device if wrong charger is used. I understand overdue fines and fees may apply if returned late, with missing items and/or damaged.
