Exam for Acceleration Application - Out of District Testing
Before completing this form, be sure that you have discussed taking an exam for acceleration with your counselor.  

Upon receipt of this form, the HPISD Assessment Department will confirm approval from your school counselor and principal to take your exam out of the district. After you complete and submit this form, you will need to complete the UT High School Credit by Exam Individual Request Form (linked on the HPISD website at https://www.hpisd.org/apps/pages/index.jsp?uREC_ID=987004&type=d&pREC_ID=1303700). Please send the form electronically (quickest method) to Dr. Jaime Callahan (callahj@hpisd.org) to obtain approval signatures before sending to UT High School. Dr. Callahan will sign the form and send it back to you so that you may then send the form to UT High School and make arrangements for online testing.
