WAIVER POLICY: I hereby give permission to Topp Tier Basketball Camp, LLC. , it's officers, employees, agents, Athletic Trainers, and staff members to take whatever action is necessary for the health and welfare of my child including consenting on my behalf to any and all medical treatment, procedures, operations and/or hospitalizations and I further agree to hold them harmless and indemnify them for all medical bills incurred for the treatment of my child.

I understand that basketball is a very physical sport and which can result in serious injury. I hold Topp Tier Basketball Camp, LLC it's officers, employees, agents, Athletic Trainers, and staff members harmless and hereby release them from liability for any injury to my child while attending camp.

Participants agrees to Topp Tier Basketball Camp, LLC., using his/her name, image, voice, likeness in any and all media, including, but not limited to print, radio, television, film, Internet and for any lawful purpose whatsoever, including editorial, advertising, promotion of the camp, and/or commercial in connection with camp-related materials.

Participants hereby agrees that he/ she will cooperate and adhere to all rules and instructions set forth by the organizers. Parent/ guardian represents and warrants that he/ she is free to enter into this agreement and has no prior or other obligations, agreements or commitments of any kind to anyone that would in any way hinder or interfere with the acceptance, and the full, uninhibited and faithful performance, of the obligations of participation. Participant represents to be in good health and condition to participate at camp.

I understand that the organizer does not provide primary medical insurance coverage for participants and that any applicable medical insurance must be provided individually by such participants.
