申請人務必填寫正確EMAIL, 當審核通過後, 會將正式領用單寄至信箱, 請將申請單印出後送所屬指導教授及實驗室負責教授簽名核准, 再憑申請單領用無塵衣. 若三個工作日未收到EMAIL. 請連絡管理者 jade46@gap.cgu.edu.tw 告知申請日期, 確認是否核准通過, 如申請之衣服尺寸無庫存, 此申請單將不會通過.
The applicant must fill in the correct EMAIL. After the review is approved, the official application form will be sent to your mailbox. Please print out the application form and ask your instructor and the laboratory host professor signature for approval. If EMAIL is not received within three working days. Please contact the administrator jade46@gap.cgu.edu.tw to inform the date of application and confirm whether it is approved. If your suit size  is not in stock, this application will not be approved.
